Right now there are about 6.000.000.000 people on this planet. If we expect everybody to get his / her own PID we should cover at least one or two more decimals (to have at least a chance within the close furture). The next question is somewhat more tricky: How devide the people (I think one easy first step could be to divide by the used language or charset (easy to find out.)) into groups. Now comes the next step: If people use the same language or charset we have to attach them to some structure: I would sugest - the date of birth as a marker that normaly does not change and that normaly is known by the patient or possible to find out (ID - cards). Further brake down should be possible by the Initial of the last and the first name, phonenumbers etc.... ... and build a personal PID number out of this informations. So my question, still open is: why would you want to attach the PID to some special kind of healthcare-provider ? If you want to define the healthcareprovider - I would kind of do the same way - first devide them by languages, then by countries, then by the typ of provider, the size of capacities and the phonenumber ...
