1. Installer didn't create ICD10 EN (english) table, so I had to
create it manually (while Installer is in progress) before
loading ICD10-EN:

2. Apache always crashes with Segmentation fault. I'm running
Ubuntu 7.10 Gutsy Gibbon JeOS edition. I've tried both XAMPP
package and also compiled PHP4 from scratch. Still Segmentation
fault. :-((
