Here in Nigeria, Referrer is not compulsory and rightly so. It's not every patient that come to the hospital that is referred. Most times, the patient just walk in. If it's a referral, the patient will come with a referral note that will be then be documented. I think the referral and Therapy field should be optional instead of mandatory.
Insurance here is also not compulsory.
