Perhaps the plugin does two things. Since casAccountDisabledView shows up in 
only 2 classes, one of which is the default login flow, I figured it had to be 
the other one.
The view is being created on line 77, 
https://github.com/apereo/cas/blob/master/support/cas-server-support-pm-webflow/src/main/java/org/apereo/cas/pm/web/flow/PasswordManagementWebflowConfigurer.java
where it looks to be part of account status.

In default login flow it is part of authentication failure (line 287), 
https://github.com/apereo/cas/blob/master/core/cas-server-core-webflow-api/src/main/java/org/apereo/cas/web/flow/configurer/DefaultLoginWebflowConfigurer.java

The particular relationship in the webflow is different in the two file and I 
no longer remember those details.

Try adding it and see what happens.
