MonoRail 2+ work for me in Integrated Mode just fine. All it takes is
the system.webServer section below. This works fine for IIS7 and IIS
Express 7. I notice that Brendan uses an older version. Would it make
a big difference?

<system.webServer>
    <httpErrors errorMode="Custom" existingResponse="PassThrough" />
    <modules>
      <add name="NHibernateSessionWebModule"
type="Yama.MVC.NHibernate.Connections.SessionWebModule,
Yama.MVC.NHibernate"/>
      <add name="routing"
type="Castle.MonoRail.Framework.Routing.RoutingModuleEx,
Castle.MonoRail.Framework"/>
      <add name="PerRequestLifestyle"
type="Castle.MicroKernel.Lifestyle.PerWebRequestLifestyleModule,
Castle.Windsor"/>
    </modules>

    <handlers>
      <clear/>
      <add name="WebResource" path="WebResource.axd" verb="GET"
type="System.Web.Handlers.AssemblyResourceLoader" />
      <add name="Trace" path="Trace.axd" verb="*"
preCondition="integratedMode" type="System.Web.Handlers.TraceHandler"/
>
      <add name="BlockConfig" path="*.config" verb="*"
preCondition="integratedMode" type="System.Web.HttpForbiddenHandler"/>
      <add name="castle" path="*.html" verb="*"
type="Castle.MonoRail.Framework.MonoRailHttpHandlerFactory,
Castle.MonoRail.Framework" modules="ManagedPipelineHandler"
scriptProcessor="" resourceType="Unspecified" requireAccess="Script"
preCondition="integratedMode"/>
      <add name="StaticF" path="*" verb="*"
type="System.Web.StaticFileHandler"/>
    </handlers>
  </system.webServer>
