I find that I also often have to:

Shut the port down connecting the phone.
Go into telephony-service, and "no create cnf"
The into voice register global and do "no create profile"
Then pick through the flash and delete any old SEP and/or SIP cnf files that 
have.a mac in the name.  
Then create profiles, create cnf-files, and then no shut the ports with phones 
attached.

Also a load file that is in the config which refers to a file that isn't 
exactly the same file name in the tftp and flash can make this happen. Check 
that you don't have (as a made up example)

Load 79xx P0S-08-09-00 

With a tftp/flash referring to a load named: 
P0S-08-9-00

You could detect this with debug tftp events.  It will be looking for a load 
that it cannot find in the tftp server.

Also, there are some old sip firmwares that CUCME hates.

Stupid question.  Does Fas0/0 have an IP address on it (maybe it is router
on a stick).  Is fas 0/0  up and up?

I would recommend that, too, except that I've noticed that it tends to work
when CUCM says it's registered but the isdn status say "tei_assigned"
instead of "multiple_frame_established".

 

I ran into this yesterday in my lab again, gets me every time.  I think that
it can be avoided by doing the CUCM confguration before the gateway's.
