Hi Dave think MoH issue might be related to a codec missmatch. Saw from you're config that the SIP phone is configured with voice-class codec. AFAIK only one codec is supported (at least in the CME version used in the lab). Therefore phone might be configured to used G.729 in your case. Would check content of .cnf.xml file.
Not sure if this does the trick, but did you play with voice register dialplan in order to get the call forwards running?
