Try to undo the AutoQOS command with the script. Then paste or write yourself the commands create by AutoQoS, if problem persist try to configure QoS without NBAR. This is just to check if is an IOS issue, Also I see you're using IPS 15.0, try it with IOS 12.4T on lasted versions.
