Why consult the wiki? http://wiki.centos.org/HowTos/Xen/MoveNative2DomU You need a xenified kernel. I see you use the plus kernel. There might be no xenified plus kernel. Anyway, what you have to do, if I remember right, is to mount the c4 partition in your c5, chroot to it and then install that xenified kernel and a new initrd to it. Read the instructions about "4.5-only" on the wiki, that should get you going.
