Do you get anything if you turn monitor debugging up to 20?
Also, would probably be nice if you could grab the output of
ceph --admin-daemon /var/run/ceph/ceph-mon.FOO.asok mon_status
and
ceph --admin-daemon /var/run/ceph/ceph-mon.FOO.asok sync_status
for each monitor (FOO being the monitor's ID)
