I have no choice except re-create this PG:
# ceph pg force_create_pg 6.9d8

But it still stuck at creating:
# ceph pg dump | grep creating
dumped all in format plain
6.9d8   0       0       0       0       0       0       0       0
creating        2014-11-09 03:27:23.611838      0'0     0:0     []
-1       []      -1      0'0     0.000000        0'0     0.000000

Do you have any suggestion?
