Number 1:

She's from the South, I think, so it's probably an insult to the
South, not blacks.  For example, it's common in some circles to hum
dueling banjos at the sight of some people, or joke about Kentucky
hill people marrying their cousin or sister.

I think it's a reference to her Southern roots and the idiot
stereotype therein;  that is, the joke is that she's smart, but Mr.
Bush always asks her to play stupid like the southern stereotype and
she willingly does it.

Maybe that's too complicated for some (southerners) to get.   j/k

No racial reference here at all.  The joke is the same as #1, she's a
smart lady who plays dumb for the President.
