Responsibility for what? Making a decision between her life and the life of 
a co-worker?

The instigator of the situation is 100% responsible for what occurs as a 
direct result of his actions, in my opinion. The gunman killed these people, 
none of the other victims involved share in his culpability in any way, 
shape, or form.
