I don't know of a specific tool that will perform both of those functions per se, but if you have a list of all variables used in the app, then you could use DWMX or cfeclipse (and probably a host of others) do directory-wide non-case sensitive to case sensitive search and replaces on variable and function calls. as for the Windows-specific directory structure I don't know if there is a tool that'll do that.
