I'd simply offer to sell them the domain name and if they refuse, let
them try to sue (assuming you actually have any paperwork to support
the former relationship with BUMC).

Most cease & desist letters are issued with on the principle that most
people won't try to fight them (Apple, for example, is infamous for
sending C&D letters at the drop of a hat). So don't view their letter
as a "threat", view it as standard legal practice.

If you don't have paperwork supporting the relationship with BUMC and
you try to fight this, any organization with Baylor in their name
could chase you for the domain (and eventually someone is going to win
unless you have very deep pockets).
