I want to ban "person X said" when they are not quoting exactly what
person X said. If they're paraphrasing someone, say so:

"a terrorist who took part in the bombing of a pizza parlor was
captured today says official"
"an activist who took part in the bombing of a pizza parlor was
captured today says official"

The first is the quote, the second is a paraphrase (if anything). This
is common in media outlets that want to remove the word terrorist for
whatever reason.

Someone will probably say that this is also common on Fox when quoting
Obama. I don't listen to Fox so I can't say but if they do, then
they're in the wrong as well.

Oh, and context along with a quote should be required, but I don't
expect that to happen anytime soon.

"I hate those white ones says Obama"
context: talking about cookies w/coconut on top
