We use this at work: http://www.phplivesupport.com. It's an open source PHP app. It's not free, though ($159/year for the bells and whistles, or $400 to 'own' the software outright). I can't make a reccomendation since I haven't had much of a chance to use it in an actuall support role (clients have enough trouble getting on and around the internet as it is), but it does have some pretty good features; http://www.phplivesupport.com/features_vs_lp.php.
