The text file is called an .FDF (Form Definition Format). We use this methodology to create quasi-dynamic pdf files for our purchasing system. The bummer is that it appears to break when going from Acrobat 5.0 to the new 6.0 version. There are also tools (Java and COM based) that dynamically create the FDFs on the fly although I haven't tried these out yet...

We were just talking about that sort of thing around here. There's another
file that you can generate that's basically a text only file that will hold
the form answers. If you have both the pdf form and the text file (I forget
what it's called) you'll end up with a filled in form.... Of course, I dont'
remember the details.
