I have a Web form at
http://valenciacc.edu/wordprocessing/orderform_west1.cfm

When the user clicks the submit button it sends an email containing the
information to the site's content manager.

The text fields on the form work fine.

But it has two problems:

1. The 'upload file' field near the bottom of the form isn't working. I
can't figure out how to get a file selected on a user's computer to be
submitted with the form.

2. If any of the checkboxes are left unchecked I get an 'element
undefined' error.

Any help or ideas are appreciated.
