People will sometimes take a random post and reply to it with a new subject, change the subject in thread or send a new email on an old subject. All of these things can cause threading problems in email readers. Even the archives which were written to deal with these issue specifically can sometimes have posts 'out of thread'. i.e. part of the same thread but listed as a new post on the thread rather than a reply.
