Just a speculation here...
Try changing this line:
<input maxlength="50" name="pol_question" type="TEXT" >
to this:
<input type="text" name="pol_question" size="50" maxlength="50">
Also, you need to split up the logic of your templates. This template is getting
difficult to read already and it's not even that big.
I recommend you looking into the Fusebox methodology: http://www.fusebox.org
