After rebuilding the server a second time, I was able to solve my CF5 / ArcIMS problem. It looks like software on the server may have been installed in the incorrect order as specified in ESRI support document http://support.esri.com/index.cfm?fa=knowledgebase.techarticles.articleS how&d=24750. Never would have thought this installation would be so tricky, but it was.
