But it's working perfectly for many users. We are using two NT boxes, balanced by
Local Director. I think that somehow, in these instances, the request for new data is
getting sent to the other box (than the one the user got the initial form from) and
the form from box A cannot be populated with the data from box B. But why?

It sounds like when you refresh you aren't submitting the data and you
aren't sending the data to the refresh, thus it's gone lost.
