These (and (2) above) are almost certainly due to the very buggy nature of CFCs in 6.0 - which all got fixed in 6.1. I would strongly recommend upgrading your production system to 6.1 a.s.a.p. It'll be faster and much more stable and CFCs will actually work properly (they really didn't work very well in 6.0).
