I'm just wondering about time. For example. I've got a file in there named
"Mail2552.cfmail". That was the first email that ever got sent. Where did
that file name come from? Will it ever be duplicated?

I think to be safe, I'm going to use the filename, the to, and the from
email addresses as the unique identifier for each email. That should be
pretty good.

Hash the contents of the file?  I don't believe the contents change as it
gets moved between Spool and Undelvr, so that should uniquely identify the
file no matter what the name.
