A VPS would have either windows or Linux as the OS...it is essentially like
having a dedicated server.  So you would have virtual sites/aliases with IIS
or apache just as you would on a dedicated server.  

The email accounts would really depend on what you install for email.  My
suggestion, though, would be to use Google apps for your email.  Having your
email on a separate and unrelated server has it's benefits...plus Google is
free ;-)  

Generally they should be firewalled on the main OS.  I don't think that is
handled at the virtual OS level.  Not sure about antivirus, but it wouldn't
hurt, I guess...unless that would interfere or not work right on a VPS...to
have one.  I would ask the provider.  

Yes, you should be able to just install it yourself with no issues, though
someone more familiar with CF licensing would be able to answer that better.
A VPS, again, is just like having a dedicated box as far as being able to
install things.

If you don't want to go to the expense of buying a license of SQL server,
might I suggest MySQL?

This is actually pretty timely...I need to simplify my current web hosting
mess.

If I had a VPS account, does that mean that I can host as many domains as I
needed (I want to centralize about a half-dozen sites)?  Or would I need an
IP address per domain?

I realize that some of these questions might differ from provider to
provider.  The sites that I would need to centralize are generally
low-traffic sites, and I think I could get by with MS SQL Server Express, a
single CPU core, and 1GB of RAM.
