Granted most of my changes are minor like updating a stored procedure or view or adding a column. I also run them on a test database first.
Updates that may have a major impact on a table with millions of records, would never be done while the sites are up, but regular minor changes and fixes by nature do not have as wide an impact as major updates and fixes.
