You would set up a scheduled task that calls code to check the mailbox, loop over the list of emails doing whatever processing you need to do and calling whatever database queries you need to call, then respond to the mail using cfmail or whatever mail sending code you prefer. You can set up the scheduled task in ColdFusion Administrator.
