We have an in house developed system for issue tracking.  It is project
based and sends out emails to all members on the mailing list for a given
project when an issue is added/modified.  Issues can be assigned to specific
people and all the usual stuff you would expect with this sort of software.
The items used to track an issue (such as priority, severity, construction
phase, etc.) are all adjustable by the project manager.

To top it off, it's web based so it doesn't need any special software other
than a browser.
