A while ago I asked around on this list about what people use for efficient paging, the answers I got was "cached queries" as far as I can remember.
I believe I finally found the "Efficient paging" I was after I posted it on my website just incase someone is interested

There was also a question a while ago (some posts just remain in the back of my head) about whether MS SQL could select specific rows like MySQL, the answer was no back then, but I believe my solution also solves that issue.
 
Unless I have gone totally nuts I believe I have the solution, I'd be happy to hear about any mistakes or other wrong assumptions I made.
