The project will integrate client-level data that is already stored within different departments. It will build on existing tools to access the integrated data for the following business functions: (a) Enabling individual client “lookup” for direct care coordination – building on Extended Client Lookup System, (b) Identifying high risk groups, based on flexible criteria, for system-level care coordination, and (c) Extracting datasets, based on flexible criteria, for analysis of population health, program evaluation and costs. Data would be made available in as timely a manner as the original datasets.
