I believe it's the former (but I don't want to speak for Dr. Carr).  I think I 
actually know a little bit about this application.  I've been dabling in the 
medical software world here as of late and have had to deal with HIPAA 
compliance in my excursions.  Point of my response is that HIPAA compliance 
is not real real hard to attain.
