Brad, I've never had a bad lifter so I can't say for sure how they are different. But I have had too much experience with exhaust leaks. Often you can feel the puff of hot exhaust, but sometimes it is between the pipe and the block, or the fan causes too much turbulence to feel it. I use a 2-3 foot section of heater hose as a stethoscope. You can put one end up to your ear, the move the other end around the exhaust system. I have had very good luck localizing leaks with this. It's not a bad mechanic's stethoscope either - just hold it against the valve cover and you can hear the valve train pretty well.
