It does go around the neck, but is much softer than a collar (ribbon) and it
is much easier to place in the correct location. When placed high on the
neck, following the jaw line up to the ears, there is not much pressure put
on the trachea like a collar would (and the dog is easier to control).

I have used show style leads on my dogs for quite some time. With Chinese
Cresteds, the hairless are also sensitive on their necks, so collars are not
an option, as with the Chihuahuas. They can be more difficult to find, so I
started making my own. I sell most styles for $5 plus shipping (actual cost
of priority + delivery confirmation). They are very easy to slip over the
head and tighten when it is time to go. I have had so many complements on
mine when I have my dogs out. I can make custom, if there is a particular
color or print (if I can find it) that you would like. I have attached a
