2. Bookmarks bar by design appears on a "New tab" page. "Always show"
unticked
doesn't mean "Never show".
3. See issue 5528 .
4. Without more detailed data this will be difficult to diagnose.
Feel free to submit separate, informative reports about 2. and 4. Just make
sure they
are not already reported, and that we can understand the problem you're
reporting.
