Visit URL 2. Enter following: PMR Number "12345,789,000" , Upload is for "IM", E-mail address "[email protected]" 3. Click Continue 4. view missing data entry boxes missing to the left of the "Browse" button What is the expected result? Display entry boxes What happens instead? No display of data entry box Please provide any additional information below. Attach a screenshot if possible.
