Believe it or not, not a whole lot exists on this. CCIE Professional
Development: Advanced IP Network Design, is a very good book on the large
scale deployment of an ISP IGP:

Of course designing an ISP involves not only the routing, but the server
back end, large scale dialup services, call center, etc. This is a void
that should be filled. Perhaps these days their are a few good books, but
alot of that clue seems to get horded.
