Since ethernet & csmacd are somewhat incidental to the evolution of the Internet (which responds to such technologies based upon their success or failure rather than any necessity-based dependencies), you might be better served by the content found in different forums, such as priscilla's certzone paper, the wildpackets/oed compendium, the IEEE 802.3 PDF (made freely available less than a month after I bought it) or any of charles spurgeon's online work at the university of texas website. A number of RFCs describe ethernet, but I haven't found any that cover the level of detail you seem to be looking for (disclaimer: I'm only on # 1287).
