        It depends on what version you currently have on your PIX, and if it's
        with (520) or without a floppy.
        
        If you have FOS version 5.1 or later, you can do it with TFTP. If you have
        an older version, you must go into monitor mode as you did.

        No, you have to go to rom monitor to do it
        
        Break into rom monitor then
        
        addr ipaddressofpix
        serv ipaddressoftftpserver
        file pix622.bin
        tftp
        
        it will load then reboot. You should then upgrade PDM, which you do the
        traditional way.
        
        I just did this a couple of days ago and it worked fine.
