When did the problem first occur or has it been occuring all along?

What changes were made in hardware or software right before the problem
started?

Is it just file transfers that have a problem or do other applications have
problems too? You said small files work, but large ones don't? In both
cases, was it FTP you were using?

What error does the user see? Does the server diplay any errors?

Are all users affected? Does it happen with any server or just a specific
server? Does it affect all operating systems?

After gathering information about symptoms and recent changes, consider a
modular approach to troubleshooting the problem. Test for problems in these
components of the network:

the LAN side
the link between the router and the CSU/DSU
the link to the demarc
service provider problems

Work your way up the OSI layers to isolate the problem:

Are cables OK? Does swapping cables help?
What does show interface display? Are there lots of CRC errors? What is the
reliability? Check the serial and Ethernet links.
What does your provider say about errors on the line?
Are you using the right clocking, signal encoding, framing, signalling,
data-link-layer encapsulation on the serial link?
Does the CSU/DSU display any errors?
On the Ethernet side, are the speed and duplex configured correctly on all
relevant interfaces?
Are you doing any sort of multilinking that could be broken?

Is network-layer addressing and routing OK?
What does show ip route display?
What do you see with debug ip packet (don't use this on a busy router,
though)

Are TCP parameters configured correctly? Use an analyzer to see what
actually happens when there's a timeout.

Are FTP parameters configured correctly?

Are there uses access issues?
Is there enough HD space to hold the large files that you are trying to copy?
Is there a firewall in the way?
