I've now fixed this completely in git; a proper /ToUnicode CMap is
created with pdftex.
However, the /ToUnicode CMap entries for the glyphs in the `v' fonts
(e.g., bsmiuv) as produced by dvipdfmx are still incorrect; to fix
this, I have to regenerate those fonts with a slightly improved
`vertical.pe' script -- I'm working on this.
