When you built sendmail from source did you ensure that you did a make
install
in the libmilter directory?
Do you have the file /usr/include/libmilter/mfapi.h?

Yes.  As I indicated, I had clamav 0.65 installed with clamav-milter 0.60p
running fine on this same machine.  So all necessary files are there, but
perhaps the new make file is looking for them in new wrong locations?
