A few notes:
~ (a)  sendmail.mc needs to be changed to reflect the socket (port 
number) instead of the path to the socket file.
~ (b)  clamd.conf can only have one socket method enabled, this is 
although clamd no longer has this limitation.  Hmm... looks like someone 
needs to modify the clamav-milter source to remove this dependency .
~ (c) so as an example:
~       --external --server=127.0.0.1 7301
