They should scan files at the same speed, but it takes clamscan longer to get started since it has to load up the virus definitions. The amount of extra time is on the order of a few seconds for a reasonably modern machine, so it's not a great concern for an entire filesystem partition scan, but do you want to use clamdscan or equivalent for mail processing....
