NAC Guest Server Hotspot Code

We are in the process of trying out a demo of the Cisco Guest Server.  In order 
to make the demo process less painful for me I was wondering if anyone 
currently using this device with the self-registration "hotspot" feature would 
be willing to share their html code for the self-registration page.
