Okay I have cleaned it all up and added the headers. Check your info
at the top to see if that is what you want used for name and email in
your contributed patches.
The mips fixes patch we don't need as the changes have been applied
upstream in gcc for a while. So I will remove that from the book. We
are closer to pushing the wip changes. Thank you to everyone for all
the contributions.
