I would not change the assessment type. It’s done and that’s that for me. We have done care conference both ways in this situation. If we don’t think the resident will be that different on return, we have gone ahead and done it on time with the social worker meeting with the resident and/or family to go over the care plan upon return. If we think they may change even more, we have waited and held care conference late upon resident return. In this case, when submitted you get a “timing” error but we just document reasoning in a progress note in case someone looks into it and have not had a problem. I have never done 2 significant changes back to back but don’t see why you can’t. Sometimes I just check Section J5a to explain the difference between MDS’s when they aren’t stable and care plan expected fluctuations. I would just do what I thought best and document, document, document!!!!! We have residents that are back and forth to the hospital with a downhill trend each time. We usually don’t actually do a significant change unless we think the change is permanent and the resident has somewhat stabilized.

I had a resident who has been in and out of the hospital, medicare A. We were doing a 5day/significant change mds. However, we completed the mds but she went to the hospital before the careplan. So what do I do? Do I submit the 5day/significant change mds? But then when she comes back from the hospital I know I do another 5day, but can I call that one a significant change too? Should I change this current 5day/sig change mds to just an MPAF? That would be extra work since it's already completed. What do I do here? She is in the hospital right now.
