He has had radiation TX. I had this question from a nurse on the unit, so I'm not positive, but I think it's a burn from the radiation.

Per the Tabers actinic skin problems are either from sun burns or radiation TX. If this man is receiving cream and has a rash.I would mark it under section M for rash with topical TX.Or is it a burn?

I hate to be ignorant but what is actinic skin changes?  I have never heard of it so I cannot offer info on how to code.

How should the following skin changes be coded on the MDS?

A recent Dermatology appointment led to the prescription of topical 5-FU for actinic skin

changes on the dorsal aspect of his hands.

Should anything be coded in M4 or should just M5 - application of ointments/medications (other than to feet) be coded?
