We use them fairly regularly with the 5 day MDS. A lot of our residents take 2-3 days to be set up with both PT and OT and will receive enough therapy for us to be paid at the Rehab high RUG and we do use grace days to be sure and capture it. We have never had a problem with our intermediary or anyone else.

It's my understanding that grace days should only be used when appropriate to place a resident in a RUGs category that accurately reflects the care he receives. I use grace days as rarely as possible. A good reason to use them would be if the resident wasn't well enough to participate in therapy the first few days after admission but is expected to be able to participate in therapy soon. I also use grace days occasionally when an admission comes late, right before the weekend and therapy doesn't get in to start treatment right away. I think that it would be a bad idea to use grace days routinely. May cause the state to review what you are doing. Janice Martin, RN MDS Coordinator Plymouth, MI |
