Just start over with a 5 day MDS (1).  I am not sure if I would do a sig change or not.  If I thought that the resident would return to previous level of functioning or continue to improve further I would probably hold off for now (00)but if something permanent had occurred such as a CVA, fracture, amputation so goals are changed etc I would do a sig change (03).

For some reason I can’t email the list serve and I have your address in my computer so I thought that I would email you with my question.  I had a resident plateau in therapy last day of PT 1/23/04.  cut letter sent, now she is going back on medicare effective 1/28/04.  do I do a sign chg assessment and restart the assessment cycle over again with the 5 day coding it 03 1 or do I continue with her previous medicare schedule and code it 03 2?  Could you get back to me with an answer soon or could you post it on the list serve for me then email me the answer?  Thanks so much.
